So here is the solution! Start by using playing cards to help you decide the size holder you want

Tilt your table saw blade to make the nameplate (front)

Tip: to save time, rip one long board so you only have to do these once

Lower your blade to about a 1/2 inch and cut slots at about 1 & 2.5 inches (measure from back)
